Has anyone used a kitchen design firm and, if so, would you recommend them? TIA

Wood Designe in Chelsea at 22nd Street did a great job for me. Diego runs the shop. The company mills its own cabinetry without a middleman, so you get 100% custom work for less than you'd expect. One of their projects was building kitchens for the Plaza Hotel conversion. The quality of work they did for me was excellent, their follow-up and attentiveness to the punch list was terrific, and it was all about 20-25% less than say MCKB for high-end finished and materials.
