Does one always require a permit from the Dept of Buildings before doing renovations on a co-op apt?

Not at all. It really depends on what you are doing. Take a bathroom for example, the plumber should get his papers in order but generally it does not have to be filed

if you live in a building ask your management company for an alteration agreement, it should tell you what you need to do
