My name is Lincoln Minott (email - minottl@optonline.net) - I am a Certified Real Estate appraiser (Lincoln Appraisal Services) doing an apptaisal at 31 Greene Street. I am requesting that a questionnaire be completed for the appraisal of apartment #5 (Maxi Cohen) - Refinance. Is there a Fax # or email address that i can send this form to be comlpeted unless your office has a generic completed form you can send me. Is there a fee of this, if yes - how much?. I can fill it out. You can send it to iwouldhitit@yahoo.com or fax to (212) 534-5322. The fee is $250, which should be left in cash in a plain paper bag in the trash can on the NW corner of E. 10th St. & Broadway.
