Do custom built-ins always add value? say next to a fireplace in a living room - bookcases and storage?

Nothing "always" adds value. If millwork is very, very topnotch flawless work, then odds are it will add value if done thoughtfully in a non-taste-specific way. If work is at all shoddy or weirdly personalized, many buyers will want to just rip it out and see it as a negative.
