Generally, when a building has a garage on site, would the tenant have to pay an extra fee to have a parking space?
I believe so. You would make an arrangement with the garage directly and pay a monthly fee for a parking space. I am not familiar with any buildings here that afford you a free parking space as part of your maintenance, but perhaps there are some buildings which do this.
There are, generally speaking, two different versions of what you are talking about. Both exist as part of the building. One is a public garage, open to everyone, and the other is dedicated just to residents. You have to pay for either. With the public version, you can often talk to them, and they may have a deal set up with the building/management. They might give you a deal that's better than what's posted on their signs as a monthly rate. The other kind, the garage that is run by and belongs only to the building, may be a little cheaper, but may also have a more fixed price, and is entirely dependant on the building's financials, availability, size, and a host of factors.
I saw one building (NE corner of 72nd & 2nd if I remember correctly) about a year ago where the maintenance included a dedicated parking space.
Generally speaking, when a building has a garage on site, it is owned an operated by another company, so yes...
What Aaron2 describes does come up every now and then, but it's quite rare (least, when the maintenance is decent). If on-site parking is important to you, this situation is something to seriously take into account.

yes. if you have to think about the expense, then get rid of your car before moving here.
