correction - interns do not work for zero - they pay to work
while the medical assistant's case sounds like the office is taking advantage of this program, internships are a way to get into a diffrent (though somewhat related) field without going back to school OR a way to avoid holes in your resume. "They should atay at home and collect unemployment." not everyone thinks like that. some people are choosing to spend their time on an endeavour that could get their foot in the door of another field, or at the very least, keep their existing skills sharp. not to mention their daily routine is keeping them from sinking into complete depression. and does it say somewhere that one cannot collect uneployment while interning? i didn't see that.

ok, just as a follow up. after extensive and arduous research on google i have learned that while it ultimately depends on the state, one CAN receive unemployment while interning. the only possible problem seems to be that one of the criteria to collect is that you must be available for work, which you are not while interning. wan't able to find any concrete answers, but this is the guide to unemployment benefits in california. it doesn't really address "internships", but on page 33 there is a section on "training programs", which is close enough. so, while it's not a definitive YES, it does suggest that it is highly probable.

some sources out there suggest not declaring at all, since unpaid interships are hard to track. yet others point out that would be fraud and not worth being caught.

not to mention that unpaid internships at for profit employers are almost always illegal.
