conflict of interest reports

What is the typical practice around the conflict of interest report requirement introduced a few years ago: https://www.nysenate.gov/legislation/laws/BSC/727 I have never received received one of these, even though I have been a shareholder since before the requirement was introduced. Are boards just ignoring this, since there is no enforcement mechanism? Has anybody ever received one? If so, was it a standalone document, embedded in the financial statement, or something else?

Every year I get a letter that states what is required by the law and is signed by each director.
