Combining with the unit below me

I'm considering buying the unit below me to create a duplex. So i'll have two floors with the same floorplan. my current unit is a 2bd/2.5bth. I have some questions. 1) How easy/difficult is it to create a stairwell? 2) Why do people say that duplexes are not worth as much? 3) If i initially don't combine the units, and take out a separate mortgage for the new unit, and then later combine, do i have to refinance the two mortgages to combine them? 4) any other thoughts regarding this combination? thanks.
